Since the Late 1800's the formal meeting process has very much been guided by the rules of order written by Henry M. Robert III, these are Roberts' Rules of Order.
These rules create a framework for the meeting process to help keep the meeting in order, it identifies the manner in which items that must be discussed are brought to the body or assembly. It defines how items are formally presented, acknowledged, maybe amended and ultimately how they are decided upon and actioned in an orderly and fair manner.

For many years things remained very much the same, meeting agenda would have been prepared and written on paper, the minutes, and voting all recorded in the same manner with pen and paper.
In time maybe microphones were added to help people hear, or even record the meetings, but realistically the records remained manually entered.
